The Youth Program Coordinator is responsible for supporting the REACH and Connect2Work Program components including enrichment, SEL (social emotional learning), academic tutoring, mentoring, and employment/educational opportunities for youth ages 14-24.

 

Major Responsibilities:

• Provide intensive one-on-one case management and navigation services to youth, prepare individual youth development plans that detail steps needed to achieve education and employments goals, conduct regular follow up to ensure participants are on-track;

• Maintain accurate and up-to-date case files documenting client goals, activities, and services;

• Document services provided and client outcomes in a case management database;

• Identify and engage local agencies for participant referrals, conduct regular outreach, and provide program orientations for recruitment into the Connect2Work and REACH program;

• Recruits, trains and places volunteers to support tutoring and job placement, mentor, and after school activities;

• Ensure all clients comply with the policies of the program and report client progress;

• Assist clients and employers as necessary with post placement issues and continuing employment needs;

• Ensures timely and accurate compliance to all reporting requirements;

• Participate in all program meetings, staff development activities, and fully engages as a member of the team;

• Comply with all policies, procedures and protocols of the agency;

• Serve as a liaison to key teachers and staff at school site, district, and community partners;

• Develop and instruct culturally appropriate and level appropriate SEL curriculum to engage students in learning and enrichment in after school programing;

• Assess and monitor students’ Social Emotional gains by regularly administrating and evaluating funder prescribed or internal assessments;

• Collect and track student data relating to the extent to which they are meeting grade level requirements;

• Maintain student attendance records and create a monthly activities schedule for all REACH programs;

• Provide instruction to support work readiness;

• Coordinate and run acculturation field trips for new arrival students;

• Ensure all clients comply with the policies of the program and report client progress including collecting eligibility documentation;

• Use IRC and program database(s) as assigned to track client services, outcomes, and metrics;

• Other related duties as assigned.
