Health Promoter

IRC in Denver’s Health Program seeks to address clients’ immediate health needs while supporting their long-term independence and access to preventive care, primary care, and any needed specialty care. The health team assists clients in accessing health screenings upon arrival or enrollment into services, connects them to a primary care provider, provides an initial introduction to the U.S. health care system, and promotes wellness by ensuring clients are educated on key health topics and supported in building knowledge and skills to access the health care system independently. The health team collaborates to deliver health services and programming under the Reception & Placement (R&P) and Matching Grant (MG) programs, the Reach program under contract with the Colorado Refugee Services Program (CRSP), and grant-funded programs.

The Health Promoter will work to improve clients’ access to health education and high quality, affordable and culturally competent health care services. The Health Promoter is responsible for helping newly arrived refugees meet their immediate and ongoing health needs by assisting them in obtaining their initial medical screenings through contracted health providers in Colorado and facilitating follow up appointments as necessary. Together with the health team and with clients, the Health Promoter supports individuals and families access and effectively utilize primary care, by providing health orientations, health-focused home visits and individualized education, culturally responsive health navigation, and transportation support and orientation. This position reports to the Health Program Coordinator and works cross-functionally to coordinate client support across internal teams and with external community partners.

MAJOR RESPONSIBILITIES:

• Transporting IRC clients to and from their medical screenings and related follow-up appointments.

• Providing customized bus orientations to new arrivals to support independent navigation to and from their primary care provider/medical home.

• Contacting clients to remind them of scheduled appointments and confirm transportation plans, providing support with transportation whether directly, by scheduling Medicaid transportation, or supporting bus route mapping and orientation.

• Assisting with patient navigation including resolving Medicaid issues and following up on referrals.

• Delivering health orientations and education, conducting health home visits, supporting cultural navigation specific to health, and providing medical case management for clients under grant-funded programs in partnership with the Colorado Health Foundation, Colorado Access, and the Colorado Department of Public Health and Environment (CDPHE).

• Together with other members of the health team, conducting client surveys to assess and address barriers to accessing primary care.

• Assisting in developing highly visual, culturally responsive health education materials to ensure the content is accessible to semi-literate, non-literate and pre-literate individuals.

• Collaborating with the Health Program Coordinator to develop and implement measurement tools that assess efficacy of health programming.

• Working closely with interpreters to assure culturally and linguistically appropriate service delivery.

• Aiding clients in developing knowledge, skills, attitudes, and behaviors needed to succeed in the U.S.

• Supporting gender equality and promote full participation of women and girls in health services.

• Maintaining physical case files and documenting case notes, entering key program data points, and completing reports accurately, timely, and in accordance with donor requirements using the IRC’s internal Efforts to Outcomes (ETO) database and other relevant systems.

• Monitoring and evaluating progress at the individual, household, and program level through analysis of client assessments and other key data tracked in ETO.

• Cultivating relationships and collaboration with colleague refugee service provider organizations to facilitate client access to holistic services and support.

• Building rapport with clients through verbal and non-verbal communication.

• Coordinating efforts with interns, volunteers, family mentors and others directly supporting clients.

• Establishing and maintain working knowledge of the R&P, MG, and Reach programs, as well as the requirements of other grants supporting the Health Program.

• Fully engaging in all program meetings, staff development activities and training opportunities.

• Complying with agency policies and procedures, as well as HIPAA and applicable federal standards.

• Performing other duties as requested.
