Health Navigator

Under the direction of the Deputy Director the Refugee Health Navigator will oversee all refugee health initiatives including navigating the U.S. healthcare system, educating refugees on various healthcare topics and acting as a liaison between the IRC in Missoula and community health partners. In addition, the Refugee Healthcare Navigator will be responsible for facilitating health access for newly resettled refugees and improve refugees’ awareness of and ability to independently access services.

RESPONSIBILITIES

• Coordinate activities outlined under Refugee Health Promotion Grant

• Assist refugees in navigating the U.S. healthcare system and educate refugees about various healthcare topics

• Improve existing relationships and generate new partnerships with additional providers to create a community that is more educated about refugee health and mental health and providing culturally competent care

• Conduct initial health assessments, coordinate health intakes to be completed within 30 days of arrival and coordinate health screening follow up appointments and referrals

• Ensure that refugees in Missoula are able to have their healthcare needs addressed and that families have guidance and support as they work towards long-term health and self-sufficiency

• Complete R&P health related case notes

• Manage community and student volunteers as Health Mentor Volunteers

• Track outcomes, collect and analyze data and comply with the monitoring and reporting to funder

• Participate in program meetings, staff development activities, and general office operations and support

• Other related duties as assigned

• Comply with all policies and protocols of the agency
