Overview of position
  • Under the current Health Programme structure, our client monitors health facilities through one provincial extender; however, there is no dedicated provision for supporting routine immunization planning, monitoring of outreach sessions, providing supportive supervision to vaccinators, or delivering systematic feedback to health facilities for corrective actions.

  • Considering the urgency of polio eradication and the need to rapidly improve routine immunization coverage, improved planning and intensified programme monitoring is required, with a specific focus on outreach session planning and implementation and community-level coverage to inform the planning and corrective actions. Our client's Health team in the southern provinces currently lacks the capacity to conduct this level of regular monitoring and support at scale. Therefore, additional provincial extenders are required to support planning and implementation, monitor outreach sessions, assess community coverage, and provide timely feedback to responsible health facilities to enable corrective actions.

Role objectives

Objective:

  • Provincial Health Extenders are responsible for performing a variety of tasks, as detailed below, to support microplanning, provide supportive supervision, monitoring service delivery and demand activities, accountability mechanisms for affected populations, facilitating structured feedback and corrective action and follow up. These responsibilities, described comprehensively in the sections that follow, do not grant any programmatic or financial authority. Provincial Extenders are selected from Afghan citizens with relevant field experience and expertise and in-depth knowledge of the geographical areas in which they operate, derived from their residency and/or professional experience.

Key tasks:

  • Support updating of EPI micro-plans at the provincial level using all available information from health facilities, the polio programme, implementing partners, and PEMTs, ensuring all villages and communities are included and that the type and number of required vaccination sessions are clearly reflected.

  • Support provinces and health facilities in preparing monthly outreach session plans.

  • Compile information on the availability of community-level resources and structures, including CHWs, Shuras, and other community organizations.

  • Develop and implement a monthly outreach session monitoring plan, with a focus on hard-to-reach areas.

  • Collect data through field monitoring and community coverage assessments using an online tool and produce a monthly qualitative report summarizing findings.

  • Review health facility performance related to EPI service implementation, coverage, cold chain functionality, vaccine stocks, and the quality of vaccines and data.

  • Provide supportive supervision to vaccinators and health facilities to strengthen capacity and improve performance.

  • Coordinate and closely work with other provincial extenders to align feedback and improvement plan

  • for the province.

  • Participate in monthly provincial review meetings to assess performance, track the status of corrective actions from previous meetings, provide feedback to PEMTs and implementing partners based on field visits, and support the development of improvement plans for the following month.

  • Submit a monthly report to the Regional Extender and our client's Health Specialist/Officer.

Project reporting

This role will work under the supervision of Health Specialist/Health Officer/M & E Specialist.

Key competencies
  • Degree/diploma in health, medicine, public health, nurse, midwife or related field.

  • A minimum of 2 years of experience in health, public health, and field work.

  • Proficiency in data analysis, interpretation and use for improving program performance.

  • Proficiency in local language; English is an asset.

  • Excellent communication skills for effective collaboration with healthcare professionals and stakeholders.

  • Ability to travel extensively in remote districts and areas.

  • Knowledge of MS office and data tools.

Team management

This role has no team management responsibility.
